GRANISETRON IMPROVES LEFT VENTRICULAR FUNCTIONS IN LEUKEMIA PATIENTS: A TISSUE DOPPLER ECHOCARDIOGRAPHIC STUDY

Objective: Nausea and vomiting are typical side effects of cytotoxic therapy. The management of nausea and vomiting has improved greatly in recent years following the introduction of the Serotonin receptor antagonists. In the light of recent developments in antiemetic care, we decided to investigate the hemodynamic effects of granisetron in terms of ventricular function utilizing conventional and tissue Doppler echocardiographic parameters. Methods: We included 22 (8 females and 14 males) patients with hematological malignancies into this study. Granisetron was given to all patients 3 mg intravenously before chemotherapy. All patients were given anthracycline lacking chemotherapy. They were studied with both conventional and tissue Doppler echocardiography before and 12± 5 hours after granisetron infusion. The mean age of the group was 40.66±15.84 years. All patients underwent a complete two-dimensional transthoracic echocardiographic, Doppler and tissue Doppler study in the left lateral decubitus position from multiple windows. Results: The conventional echocardiographic outcomes were all found identical before and after chemotherapy (p>0.05). Left ventricular ejection fraction (64.66±2.26% vs 66.66±2.93%, p=0.002), fractional shortening (35.62±2.09 vs 37.06±2.64, p=0.008), mitral lateral S wave (10.01±1.79 vs 11.96±1.87, p=0.04), mitral lateral E wave (10.92±2.36 vs 12.04±1.97, p=0.04), mitral septal S wave (8.64±2.02 vs 12.11±2.17, p=0.05), mitral septal E wave (11.12±4.08 vs 13.25±2.89, p=0.0011), right ventricle lateral E wave (14.33±2.45 vs 15.7±2.68, p=0.0006) showed beneficial results with iv granisetron application. Conclusion: We can conclude that granisetron, which is used as an antiemetic agent, slightly improves the left ventricular systolic function and passive diastolic filling of both ventricles in cases with hematological malignancies.
