Surgical Treatment of Peripheral Pseudoaneurysms

Özet
BAC K G R O U N D N i n e t y-four patients operated due to the diagnosis of pseudoaneurysm between January 1985 and January 2007 have been respectively studied. M ET H O D S Fifty-six of the cases were male (59.6%), and 38 were fem ale (40.4%). The most freq uent findings were puls atil mass and the thrill on pseu d oa n eurysm. The dia gn osis was conf i rmed by class ical ang io graphy, Doppler ult r as on o graphy and magn etic res on a nce ang io graphy. R E S U LT S The loc at ion of the pseudoaneurysms were as follows: 50 superfic ial fem oral artery (61.7%), 14 brach ial artery (14.9%), 8 common femoral artery (8.5%), 8 popl it eal artery (8.5%), 6 rad ial artery (6.4%), 4 ulnar artery (4.3%), 2 prof u nda fem oral artery (2.1%), 2 int e rnal car otid artery (2.1%). In etio l og ical exam in at ion, pen e tr ating trauma in 61 cases (64.9%) and blunt trauma in 22 cases (23.4%) w ere obs e rved. The dur at ion betw een the trauma and pseudoaneurysms surgery ranged from 4 days to 7 years. In add it ion to aneu r ys m e ctomy in the surg ical proc e s s , graft int e rp os it ion with aut og en ously vein in 45 cas e s (47.9%), (saph en ous vein in 42 cases, and cephalic vein in 3 cases) were preferred. Primary rest or at ion using dir e c t r ep air in 24 cases (25.5%), end-to-end anast om osis in 20 c ases (21.3%) and lig at ion in 4 cases (4.2%) were applied. Polytetrafluoroethylene graft was used in 1 case (1.0%). T h r ee amp ut at ions (3.2%) and 2 mort ality (2.1%) occ u rred during hosp it al iz at ion. 41 pat ients (43.6%) were foll owed-up for 2-60 months per iod. The med ian follow- up p er iod is 7 months. Recurrence was found in 5 patients (5.3%). The mean time of recurrence is one mount. All of these rec u rr e nces were treated by rea n eu r ys m e ctomy. The pat ients gained good funct i onal rec overy. CO N C L U S I O N S Early restorative operations have positive effects on long term prognosis and best functional results by choosing a suitable operation method before the complications develop in pseudoaneurysms.
